Abstract

PT. X is an automotive indutsry produces front and back lamps for motorcycles and cars. Production processes are divided into injection, aluminizing, and assembling. In the production process, PT. X often meet failure that cause in producing under specification and standard product, and categorized as defect product. All the products’ defect percentage are above the assigned percentage that is 2%. To break all the problems above, Failure Modes and Effect Analysis (FMEA) is used. And also some other tools to help solving the problems such as Fault Tree Analysis (FTA), Fishbone Diagram, and Pareto Diagram. The step began with processing production data in 2015. It helps to discover which process that experience failure and causing much defect and to discover causative factors. After all the steps were done, it shows that aluminizing process is the one that often experience failure with defect percentage 66,3 %. Dot defect is the most appeared defect with 47,3 % percentage. Causative factor of dot defect in aluminizing process is because the machine and tool. Machine often meet error (fan belt motor for exhaust blower is broken) because of the continous usage and jig and inside part of the machine is filthy because of the high usage intensity. Calculation using FMEA method results RPN in aluminizing process is 512 and placed at the first rank. Therefore, aluminizing process become priority to be fixed. The possible improvement to do is making periodic schedule in exchanging fan belt motor for exhaust blower, and improvement in jig cleaning schedule and inside part of the machine, which has always been done once a week become once in 4 days.
